Discharging from an acute setting back to a care home after 5pm on a Friday and over the weekend

NHSE in preparation for Winter 2018 want to gather your thoughts in respect of discharging people from an acute setting back to a care home after 5pm on a Friday and over the weekend.

They would like to understand how both they and care providers can work more collaboratively to ensure communications; processes and support are in place to enable people to come back to their usual place of residence.

The Ask:
To this end they would like to hear about solutions that could improve that which takes place.

NHSE are aware that some providers due to experience have chosen not to accept discharges from Friday after 5pm through to Monday morning.

The aim would be to produce a communication for all parties to consider so that delayed transfers of care could be avoided where possible and access to care homes could take place over the weekends.

Your thoughts of this topic would be greatly appreciated no later than 19.09.2018.
