A quarter of the care homes in Buckinghamshire require improvement according to charity, Independent Age.
At the other end of the scale, just 3.9% are outstanding.
They say recruitment struggles and not enough support for staff is having an effect and is calling for both councils and the Government to act.
This research also shows, for the first time, a year-on-year comparison on care home performance across regions and local authorities in England.
